Just like with every Dragon Incantation, Rotten Breath is unlocked with Dragon Hearts. As the name might imply, these items are dropped by Dragons upon defeat, but don't just go around killing Elden Ring's ancient enemies willy-nilly. You only need one Dragon Heart to get Rotten Breath, after all.
Where to find Mushroom Set in Elden Ring. The Mushroom Crown is found on a corpse hanging off of a pillar ruin in the south-eastern corner of Lake of Rot. The rest of the set is found in Seethewater Cave, in a cavern with a Giant Poison Flower and multiple mushroom people.
